FLU SHOT Q & A
Q. Are the seasonal flu
shots from Walgreens reimbursable under OCC's PEP program?
A. Yes.
However, employees will be required to file a claim with their
health insurance carrier to determine if the flu shot expense will
be covered. This should be done before requesting reimbursement
under the Physical Exam Program. OCC will cover out-of-pocket
expenses associated with a flu shot that was not covered under an
employee's health insurance up to $15. The following should be
submitted when filing for reimbursement under the PEP program.
-
PEP
Reimbursement Request form
-
A
completed and signed HIPAA Authorization Form
-
Explanation of Benefit statement(s) pertaining to my annual
physical exam
-
Copies of
corresponding medical bills, statements, or payment receipts

Q. Is the maximum amount
still $15?
A. Yes.
Q. Will I have to file the
"Walgreens flu shot" receipt with my health insurance first, to
obtain an EOB, prior to submitting it for a PEP claim? Or can I
just mail in the receipt?
A. See
response to the first question.
Q. Will OCC cover BOTH the
seasonal flu shot (available right now) as well as the new swine flu
shot (available in a month or two), or just $15 per year?
A. Flu shot -
see above response.
A. H1N1
vaccine - The OCC has not made a determination on how the cost for
the H1N1 vaccine will be handled. This is currently under
discussion as we are trying to get an understanding of how the H1N1
vaccine will be handled overall for the federal government.
